Care Management
Care Management Program
Care management is a sustainability program which examines an individual’s emergent needs and ability to age in place. The three areas of focus are (1) current health status and medical needs, (2) nutritional aptitude and condition, (3) home safety. Individualized care plans are developed to maintaining clients in the least restrictive environment. The Care Management Team (CMT) examines both informal and formal supports to develop the client’s supportive circle. CMT will work with the client for 120 days maximum. The CMT also works closely with Adult Protective Services to provide support to elders who may be at risk.
Caring Companion Program
Caring Companions are volunteers 18 years or older developing helping relationships that empowering elders to age in place. They provide friendship and support services to clients who may be lonely, ill, or disabled. Services of a Caring Companion may include: accompanying their clients to medical appointments, the grocery store, senior community centers or social events. Without the help of Caring Companions, many older adults would not be able to continue living at home and would need more expensive, less personal institutional care.
HCBS Case Management
The Home and Community Based Services program allows people, who would otherwise be institutionalized, to live in their own home or in the community. The services are made available through the Department of Public Health & Human Services (DPHHS) and the Senior and Long Term Care Division (SLTC). Our Case Management Team (CMT) consists of a Registered Nurse and Social Worker who provide a holistic approach to care planning. They look at each individual’s medical and psychosocial needs and then develop a plan of care based on the person’s needs and choices. Each Case Management Team has a fixed number of individuals they can serve per year. There is often a waiting list for this program.
Senior Companion program
Senior Companions (SC) are healthy older adults who help other adults and seniors live independently. They provide friendship and support services to clients who may be lonely, ill, or disabled. Services of a Senior Companion may include: Accompanying their clients to medical appointments, the grocery store, senior or community centers or social events. SC’s may assist their clients in obtaining needed social services and completing daily tasks. Without the help of SC’s, many older adults would not be able to continue living at home and would need more expensive, less personal care.
Respite Program
Respite Program Case manager works one-on-one with the primary caregiver and their families to create a holistic program which provides support, advocacy, education and resources. The respite program offers assistance to help identify formal and informal support systems, which can provide an alternative caregiver which allow a temporary period of rest or relief (respite) to the primary caregiver. Respite services helps to reduce stress and improve the health and overall well-being of the primary caregiver while also enhancing the care recipient’s quality of life.
Respite Companion program
Respite Companions are volunteers 18 and older who provide on-going weekly companionship and care while sharing their unique talents and interests with an older adult. This support allows the primary caregiver a period of relief (respite) from their day-to-day caregiving responsibilities.
